ST JOSEPH'S HEALTH CENTRE FOUNDATION OF TORONTO

A strong community needs great health care close to home. Thanks to the generosity of donors, the Foundation supports the Health Centre’s remarkable physicians and staff as they take care of our community. Donations support facility renovations to our patient care areas. For example, support from donors helped build a new wing that opened in 2012. Donors also help purchase essential medical equipment, like a new endo-bronchial ultrasound that pinpoints lung cancer, and funds training for our dedicated health care professionals. Current priorities include renovating our patient spaces like lounges and waiting areas to make the more comfortable for patients and families; building a new Dialysis Centre; and building and equipping two new Interventional Suites in our Diagnostic Imaging department.

ST. JOSEPH'S HEALTHCARE FOUNDATION, HAMILTON

St. Joseph's Healthcare Foundation is known for building lasting, mutually rewarding partnerships that enable St. Joseph's Healthcare Hamilton, a multi-campus, teaching hospital, to provide the best in compassionate patient care and innovative medical research. In June 2011, the Foundation publicly launched Timeless Care, Tomorrow’s Discoveries – The Campaign for St. Joseph’s Healthcare, which will raise $75 million to support three vital projects at the hospital: the opening of a state-of-the-art surgical centre on the Charlton Campus; the creation of a new integrated mental health and medical care facility on the West 5th Campus; and the funding of vital healthcare research at St. Joe’s. These projects will touch the lives of two million Ontarians who live in the region served by St. Joseph’s.

VICTORIA HOSPICE SOCIETY

Victoria Hospice is a registered charity that has, since 1980, provided end-of-life care focused on palliative treatment. Our nurses, counsellors, spiritual caregivers, physicians and trained volunteers provide comfort for the patient and support for the family, rather than a cure for progressive and life-limiting illness. Care is offered in patients’ homes, within our 17-bed in-patient unit, and as specific medical consultation to patients in other health care facilities. Over 300 trained volunteers provide companionship and support. Victoria Hospice offers bereavement support to families for up to one year following a death. Roughly 50% of Victoria Hospice's annual operating costs are not covered by our health care system, and must be funded through community donations to our Victoria Hospice and Palliative Care Foundation.

Canadian Mental Health Association - Calgary Region

We provide caring, community-based support programs like supportive and independent living, homeless outreach, recreation, suicide bereavement support, and peer support programs for families and individuals. As well, we work closely with other community organizations to bridge gaps in the mental health care system and facilitate access to important services and resources. One in five people experience a mental disorder in their lifetimes and most of us know someone who has faced a mental health challenge or concern. CMHA – Calgary Region strives to improve public understanding of mental health issues at home, in the community, and workplace through community education, advocacy and mental health awareness initiatives. TROtt Therapeutic Riding Association of Ottawa-Carleton

Located at Equidae Stables, 6362 Bank Street in Greely, TROtt has been serving a wide range of riders with mental, physical and emotional conditions for more than 25 years. Riders with a varied range of conditions can participate in the TROtt program e.g. cerebral palsy, spina bifida, stroke, head injuries, multiple sclerosis, visual impairments and mental difficulties. TROtt operates from March to December, using indoor and outdoor facilities. Lessons are scheduled during the day, evenings and Saturdays

Fertility Matters Canada

It is our mission to support and educate couples suffering from infertility so that they can make dignified choices as they proceed on their journey. We have a 1-800 support line, publish the only Canadian quarterly magazine about infertility, have our own website which displays a wealth of information and a patient forum. We also have 30 volunteer Chapter Leaders who are operating support groups from coast to coast and we have public service announcements being aired on the radio from coast to coast.

CHILDREN'S MENTAL HEALTH ONTARIO

CMHO works with many partners who deliver treatment and prevention services to children, youth and their families. Our core membership consists of accredited children’s mental health centres, but we also work with child welfare agencies, schools, early childhood centres, family physicians and pediatricians, hospitals, adult mental health services, and other organizations that work to improve the lives of young Ontarians and families affected by mental illness. Our goals are to: - promote service excellence and innovation through standards and accreditation, dissemination of evidence-based practices, and information sharing and education; - enhance the capacity of children’s mental health service providers to offer optimal services for children, youth and their families; - advocate for policies, programs and funds to improve the system of children's mental health services; and - support the work of parents and board members in children's mental health.

ALZHEIMER SOCIETY ALBERTA & NORTHWEST TERRITORIES

The Alzheimer Society of Alberta and Northwest Territories is a charitable organization with 5 key service points and 3 light service points in Alberta and Northwest Territories. We do not receive any ongoing(core) government funding, and close to 70% of our revenue comes thanks to individual donors, with another 20% coming from our valued corporate partners. Another 10% revenue comes from interest and the occasional government grant or casino revenue. WHAT WE DO: Our programs and services are offered throughout Alberta and into the Northwest Territories. We also fund research into the causes and the cure for Alzheimer disease. We provide information and publications, deliver public education forums, provide personal and confidential consultations, and coordinate various support groups across the province to those affected by dementia, their family and caregivers. Onsite information sessions at places of employment are also offered in the community.

The Alzheimer Society provides services to persons with Alzheimer Disease and related dementias and their caregivers that include: Advocacy, caring and understanding support systems for individuals, families, in both individual and group formats ; educational services that includes an individual and group sessions for those directly and indirectly impacted by Alzheimer's disease or a related dementia, a lending library and newsletter; volunteer in-home respite service; professional in-service training Client services are provided at no cost. The Mood Disorders Association of Manitoba is a self-help, peer support organization, offering on-site and phone peer support, education, and support groups to people living with mood disorders and their family, friends and loved ones. We also provdie on-site education for any persons, organizations, employers etc. who wish to know more about mood disorders, what they are, how they are treated, and how to assist persons with these disorders.
