ALBERTA DIABETES FOUNDATION

Alberta Diabetes Foundation (formerly AFDR (Alberta Foundation for Diabetes Research) was organized by a group of volunteers, some of whom were parents of Diabetics or in other ways affected by Diabetes. The Alberta Foundation for Diabetes Research was incorporated under the Societies Act on Nov. 4, 1988. Alberta Diabetes Foundation's purpose is to raise funds to support world-leading diabetes research projects. Besides funding diabetes research, Alberta Diabetes Foundation works with the researchers to provide our supporters with up-to-date information about what's happening on the research front. We also strive to create opportunities for interaction with other diabetics. Alberta Diabetes Foundation hosts many promotional events and can organize lab tours of current projects.

CDHF - Canadian Digestive Health Foundation

Over 20 million Canadians suffer from digestive disorders every year. We believes this is unnecessary and unacceptable. The CDHF reduces suffering and improvs quality of life by empowering Canadians with trusted, up to date, science-based information about digestive health and disease. We are the Foundation of the Canadian Association of Gastroenterology (CAG). As such, we work directly with leading physicians, scientists, and other healthcare professionals to help Canadians understand and take control of their digestive health with confidence and optimism. Through research and public education, we aim to: REDUCE the incidence and prevalence of digestive disorders; IMPROVE understanding of digestive health issues; SUPPORT those suffering from digestive disorders; and, ENHANCE quality of life for those living with digestive disorders.

HABITAT FOR HUMANITY HURONIA

We provide safe, affordable, decent home ownership to deserving families in Barrie and South Simcoe. Our Restore recycles usable building products and materials diverting more than 40 tons of waste each year. Low-income families complete 500 hours of sweat equity toward their home and pay a no interest mortgage. They become tax-payers and engaged community citizens. Our ReStores and building projects engage more than 400 volunteers every year including students, Ontario Works participants, volunteers through the justice system and other community minded individuals. Thanks to Restore revenues, 100% of donations go directly to helping families in need get a hand up not a hand out.

HALIBURTON HIGHLANDS HEALTH SERVICES FOUNDATION

The Haliburton Highlands Health Service Foundation is an incorporated registered Canadian charity that financially supports the hospitals and health care facilities in Haliburton County. The money is raised through personal, business & corporate donations, special events, promotions, annual giving and the special gift program. The Foundation is governed by a volunteer Board of Directors who dedicate their time and expertise to have better health care and hospital facilities.

HURON HOSPICE

A dedicated team of Volunteers provide companionship, listening and emotional support to residents of our community who are facing a life threatening illness or injury. The Volunteers offer relief to enable primary caregivers to feel comfortable to take time away or rest. This care can be provided in a home, a hospital, or a long-term care setting. There are NO FEES for these services. Our Volunteers are people who display sensitivity, good communication skills, and willingness to help and have respect for the privacy, dignity and self-determination of the person. They are carefully screened and have completed an approved Volunteer Training Progrm. They receive ongoing education, supervision and group support. Agency governacy is by a volunteer Board of Directors which is drawn from all our communities.

HOSPICE HUNTSVILLE

Hospice Huntsville provides care and support to individuals in the Huntsville area who are facing end of life situations. This care and support is also extended to their families and caregivers and is provided in the setting of their choice. Our professionally trained volunteers provide these individuals with compassionate care so that they may live out their lives with dignity and comfort either at home or in a health care setting. Hospice Huntsville is a not for profit registered charitable organization, incorporated in 1989. It is primarily funded by voluntary donations, churches, service clubs, in memoriams, planned fund-raising and some government funding. All services of Hospice Huntsville are offered at no cost to those we serve.

Lyme Ontario works with patients and researchers to obtain and provide scientific, peer-reviewed information on Lyme disease and other tick-borne infections such as Babesiosis, Erhlichiosis and Bartonella.

PHILIP AZIZ CENTRE FOR HOSPICE CARE / Emily's House

When there is no cure, there is still support, care and compassion.....there is hospice. This is one of Philip Aziz Centre's key messages as a Hospice organization to make sure that no one (patient or their caregivers) should have to journey alone with a serious illness. This is why we advocate for, and efficiently deliver, quality, timely hospice care that is a necessary part of our health care system.Expanding upon our 17 years of hospice care, Emily's House , a 10-bed, family centred children's hospice, the first in the GTA, will open in January 2013.This hospice will service children and their families across Ontario who require a third care alternative to a hospital bed or 24/7 home care, which is not always available or can just be too much for the families to deal with. Whether a life is measured in days,weeks, months or even years, children living with a life-limiting diagnosis deserve the oportunity to learn, play, develop and grow....to just be kids.

Alzheimer Society Foundation of Brant, Haldimand Norfolk, Hamilton Halton

The Hamilton and Halton Alzheimer Foundation fundraises on behalf of the Alzheimer Society of Hamilton and Halton, and administers all funds raised. This Foundation and The Alzheimer Society of Hamilton and Halton are under the direction of a common volunteer Board of Directors. The Board works with the Society's staff and its community partners to achieve its fundraising objectives. Huntsville District Memorial Hospital Foundation is a fundraising organization dedicated to improving health care services for the residents of Muskoka and East Parry Sound. The Huntsville District Memorial Hospital Foundation was established as a registered charity in 1984 to receive, maintain and distribute funds raised by personal donations, grants, capital projects and special event fundraising, and special programs not covered by government grants. Although the Foundation does not contribute to operating costs, its mandate is to provide ongoing capital and education resources for Muskoka Algonquin Healthcare - Huntsville District Memorial Hospital.
