At LifePath Hospice, we go way beyond those basic hospice requirements to identify and meet the specific needs of each patient and family—from extra homelike touches in our two hospice houses to the hands-on bereavement care we provide to families long after their loved one has died. The mission of LifePath Hospice is to provide quality palliative care and relieve the suffering of those in our communities affected by life-limiting illnesses and end-of-life issues, maintaining the highest ethical standards, so all may live as fully and comfortably as possible.

ORGANIZED FOR CHARITABLE AND EDUCATIONAL PURPOSES TO PROVIDE SUPPORT AND MEDICAL CARE FOR PERSONS WHO ARE TERMINALLY ILL BY OPERATING AN IN-PATIENT FACILITY WITH A HOME LIKE ATMOSPHERE FOR HOSPICE PATIENTS. INDIVIDUALS WILL BE ADMITTED WITHOUT REGARD TO RACE, COLOR, RELIGION, SEX, AGE, SEXUAL ORIENTATION, HANDICAP OR ABILITY TO PAY.

THE ORGANIZATION'S MISSION IS TO CARE FOR ABUSED, NEGLECTED AND AT-RISK CHILDREN AS WELL AS THOSE WITH DEVELOPMENTAL DISABILITIES IN THE JEWISH COMMUNITY AND TO WORK IN PARTNERSHIP WITH FAMILIES AND THE ENTIRE COMMUNITY.

ESTABLISHED IN 1978, THE CENTER FOR HOSPICE CARE (CHC) IS AN INDEPENDENT, COMMUNITY BASED, NOT-FOR-PROFIT ORGANIZATION, IMPROVING THE QUALITY OF LIVING THROUGH HOSPICE, HOME HEALTH, GRIEF COUNSELING, AND COMMUNITY EDUCATION WITH OFFICES IN SOUTH BEND, PLYMOUTH, ELKHART, LA PORTE AND MISHAWAKA. CHC SERVES A NINE-COUNTY AREA IN NORTHERN INDIANA, INCLUDING ELKHART, FULTON, KOSCIUSKO, LAGRANGE, LA PORTE, MARSHALL, ST. JOSEPH, STARKE, AND PORTER COUNTIES. CHC'S MISSION IS TO IMPROVE THE QUALITY OF LIVING THROUGHOUT NORTHERN INDIANA, AND OUR VISION IS TO BE THE PREMIERE HOSPICE AND PALLIATIVE CARE ORGANIZATION FOR ALL END-OF-LIFE ISSUES. OUR CORE VALUES INCLUDE COMPASSION, DIGNITY, INTEGRITY, INNOVATION, QUALITY, SERVICE, AND STEWARDSHIP.

BC Children's Hospital Foundation raises money to support excellence and innovation in care, treatment and research and BC Children's Hospital, Sunny Hill Health Centre for Children and the Child & Family Research Institute. The Foundation's activities include major gift fundraising, planned giving and a wide variety of annual programs. As steward of philanthropic donations to Children's Hospital, Sunny Hill Health Centre and the Child & Family Research Institute, the Foundation issues charitable receipts and ensures donations are allocated to the areas to which donors have designated their support. The Foundation is currently engaged in the $200-million Campaign for BC Children to build a new children's hospital and ensure all BC children have access to a consistent standard of pediatric care.

Our two major programs provide hope for the future through the support of MS research into the cause, treatment and cure of the disease and hope for today through our many services that assist people with MS and their families. Since our founding in 1948, the core support of the MS Society has been from tens of thousands of dedicated individuals, companies and foundations in communities across Canada. The Society receives almost no funding from government.

United Cerebral Palsy (UCP) educates, advocates and, provides support services to ensure a life without limits for people with a wide range of disabilities. UCP and its nearly 80 affiliates advance the independence, productivity and full citizenship of people with a spectrum of disabilities by providing services and support to more than 176,000 children and adults every day-one person at a time, one family at a time. We work to enact system change-to revolutionize care, raise standards of living and create opportunities-impacting the lives of millions living with disabilities. UCP was founded in 1949 by parents of children with cerebral palsy. For more than 60 years, UCP has worked to ensure the inclusion of individuals with disabilities in every facet of society. Together, with parents and caregivers, we will continue to push for the social, legal and technological changes that increase accessibility and independence, allowing people with disabilities to dream their own dreams, for the next 60 years, and beyond.

Founded in 2015 by Juliet de Baubigny, Nick Jonas, Sarah Lucas + Sam Talbot, Beyond Type 1 is leveraging the power of social media and technology, changing what it means to live with a chronic disease. By educating the global community about this autoimmune disease, as well as providing resources and supporting those living with Type 1 diabetes, Beyond Type 1 is bridging the gap from diagnosis to cure, empowering people to live well today and funding a better tomorrow.

Surrey Women’s Centre offers crisis, court and counselling services for women and children escaping domestic violence, sexual assault, child abuse and other forms of gender-based violence. We are available 24 hours a day, 7 days a week, 365 days a year to ensure that women and children have the support they need, when they need it!

The Christopher and Dana Reeve Foundation is dedicated to curing spinal cord injury by funding innovative research, and improving the quality of life for individuals living with paralysis through grants, information and advocacy.
